I respectfully ask that you submit the following Covid-19 Health declaration within 24 hours prior to your appointment.

Please reschedule if: 

• You or someone in your household has been recently exposed to Covid-19 or are awaiting test results.

• You or anyone in your household are experiencing cold/flu-like symptoms.

• You have been exposed to someone testing positive for Covid-19 in the last 14 days.

• You have been traveling within the last 21 days.

You will be required to wear a MASK covering your nose and mouth for the duration of your visit.

 

At this time some services may be unavailable.
